Squadmates! its me... cheryl... i noe tat tym jess giv tat form tat tym not enuf and some of u all dun hav. chu jing supposed 2 scan and email u all... but i noe so far she hasnt and some of u hav been asking u for it... so i decided to scan it and email it to u... i noe it has been signed by my mum... but i tink u all wont mind der rite? tape over and photocopy agn jiu can le ma ... rather than u hav nth at all... better than nth rite... there is one copy send 2 u by email and another posted on this blog. so those hu nid it, go find some way 2 get a copy.
